First, Your employer sets certain health criteria it wants to reward. Download your PointsPassport and see if you meet the criteria to meet points.
The next step is to fill out your Points Passport. You can scan it and upload it via a secure web form, or you may fax it to us as well.
If meet these criteria, you can then be eligible for rewards in the form of discounts on your premium.
